| MINIMUM ORDER: | $250.00 $ 20.00 charge for less than minimum order. |

| PAYMENT TERMS: | Net 30 days. Interest may be charged at the rate of 1.5% per month on any overdue balance. |
| FREIGHT POLICY: | For shipments within continental U.S.A....FOB: Orangeburg, NY Over $1,500.00: No freight charge |
| FREIGHT SURCHARGE: | 4% Surcharge will be added for shipments to the following states: AZ, CA, CO, ID, MT, ND, NM, NV, OR, SD, UT, WA, WY |
| DROP SHIPMENTS: | Drop shipments to one destination will be subject to a $20.00 service charge. Regular freight terms apply |
| CARRIER CLAIMS: | Claims for defective shipping cartons and shortages in shipment must be filed by the customer and noted on the carrier’s freight receipt at the time of delivery. |
| RETURNED GOODS POLICY | |
| AUTHORIZATION: | Written request must be sent to Dynarex listing Dynarex Invoice number before any merchandise can be returned. Written authorization will be given by Dynarex prior to return of merchandise. |
| MERCHANDISE RETURNABLE FOR CREDIT: |
Credit will be issued on the original net purchase price. Only unopened cartons will qualify for return. |
| FREIGHT AND RETURNED GOODS CREDIT: |
Unless otherwise stated in writing, all returns shall be shipped prepaid to Dynarex warehouse. Credit will be granted as follows:
